About the Role
Assists Case Managers in resident interviews to determine appropriateness for residency and identifies/evaluates resident needs, interests, and strengths. Ensures the completion and updates of Individual Service Plans (ISP) and care plans. Performs assigned nursing procedures for resident comfort and well-being, administers medications, and monitors resident conditions.
Responsibilities
- Assists Case Managers in the initial interview of enriched housing residents to determine appropriateness for residency.
- Works with Case Managers to identify and evaluate resident needs, interests, and strengths.
- Documents resident needs and capabilities on the Individual Service Plan (ISP).
- Assures updates and completion of the ISP and care plan.
- Performs assigned nursing procedures under the direction of the RN.
- Administers and documents physician-ordered medications accurately and timely.
- Follows facility procedures for ordering and receiving medications.
- Transcribes physician orders and ensures proper interpretation and execution.
- Monitors residents' conditions, documents changes, and refers problems to the physician and RN.
- Communicates changes in resident conditions to family representatives as appropriate.
- Establishes work assignments for HHAs and supervises their task completion.
- Assists residents with bathing, dressing, or grooming services as needed.
- Disposes of discontinued/expired medications according to facility policy.
- Maintains medication carts clean, organized, and maintained.
- Performs quality assurance audits as directed.
- Maintains documentation of shift occurrences for thorough reporting.
- Notifies RN/Supervising Nurse of resident needs, including wound care, dressings, treatments, and skin concerns.
- Notifies RN of any concerns/changes in care needs expressed or noted by any resident.
- Assists in training new staff on their assigned shift.
- Performs duties according to ethical standards and the facility's corporate compliance plan.
- Adheres to the facility's confidentiality statement regarding protected health information (PHI).
- Completes a second check of all physician/provider orders for accuracy.
- Prints and reviews daily Electronic Medical Record (EMR) reports as assigned.
- Reviews daily residents on wellness checks in EMR and ensures frequency and stop dates are completed.
- Performs other duties as assigned by RN or Vice President of Assisted Living Services.
Requirements
- Graduate of an approved school for licensed practical nursing.
- Currently licensed in New York State to practice nursing as an LPN.
- Leadership ability.
