About the Role

VHA Home HealthCare is seeking a Dietitian to assess and provide consultation and recommendations for dietetic interventions to adults, geriatric, and/or pediatric clients in their homes. The role involves ensuring coordinated care through communication with the client care team, problem-solving for client independence, and promoting VHA’s core beliefs and values.

Responsibilities

  • Prepares client reports for health professionals to analyze for treatment.
  • Prepares plans and presentations for clients and their caregivers on diet and goals.
  • Educates internal and external staff members about healthy eating and proper nutrition.
  • Supports intervention areas including failure to thrive, dysphasia, feeding problems, enteral feeding, diabetes, cancer, GI disorders, weight management, and palliative care.
  • Helps patients cope with their illness and advises on special and time-specific diets.
  • Supports safe client care through continuous quality improvement.

Requirements

  • Member in good standing with the College of Dietitians of Ontario.
  • Minimum 2 years clinical experience (optimal).
  • Valid Driver’s License with access to a vehicle.
  • Compassionate and empathetic.
  • Self-driven with ability to work independently.
  • Skilled in caseload management.
  • Skilled at electronic documentation.
  • Able to provide virtual care.
