About the Role
VHA Home HealthCare is seeking a Dietitian to assess and provide consultation and recommendations for dietetic interventions to adults, geriatric, and/or pediatric clients in their homes. The role involves ensuring coordinated care through communication with the client care team, problem-solving for client independence, and promoting VHA’s core beliefs and values.
Responsibilities
- Prepares client reports for health professionals to analyze for treatment.
- Prepares plans and presentations for clients and their caregivers on diet and goals.
- Educates internal and external staff members about healthy eating and proper nutrition.
- Supports intervention areas including failure to thrive, dysphasia, feeding problems, enteral feeding, diabetes, cancer, GI disorders, weight management, and palliative care.
- Helps patients cope with their illness and advises on special and time-specific diets.
- Supports safe client care through continuous quality improvement.
Requirements
- Member in good standing with the College of Dietitians of Ontario.
- Minimum 2 years clinical experience (optimal).
- Valid Driver’s License with access to a vehicle.
- Compassionate and empathetic.
- Self-driven with ability to work independently.
- Skilled in caseload management.
- Skilled at electronic documentation.
- Able to provide virtual care.
