About the Role

The Peer Health Navigator will connect Intensive Crisis Residence (ICR) guests to community resources, aiding their transition through crisis and return to community life. This role is crucial for successful discharge planning, helping guests establish and maintain connections with professional and community-based supports. Services extend for 90 days post-discharge to ensure stability.

Responsibilities

• Connect ICR guests to community resources for support during crisis and reintegration.
• Facilitate the establishment and sustainability of connections to professional and community-based supports.
• Provide peer navigation services for 90 days post-discharge to ensure crisis resolution and stability.
• Ensure all guests are offered Peer Health Navigation Services.
• Enter all progress notes into AWARDS within 48 hours of meetings.

Requirements

• Understanding, appreciation, and commitment to the philosophy and mission of Community Access.
• New York State Peer Certification (required or attained within the first year of employment).
• High School Diploma or equivalent (required).
• Ability to prioritize and meet deadlines.
• Strong analytical ability.
• Excellent oral and written communication skills.
• Creativity and flexibility.
• Ability to utilize various computer programs including Microsoft Word, Excel, and cloud-based data management platforms.
• Initiative and responsibility for follow-through.
• Ability to maintain confidential information.
• Ability to work independently and as part of a team.

• The ICR works with guests for up to 28 days to develop skills for community reintegration and managing mental health concerns.
• The program follows the Soteria model emphasizing shared home environment, positive recovery expectations, validation of experiences, 'being with' a person, 1:1 support, and self-determination.
• Services are voluntary, and guests are encouraged to maintain community responsibilities.
• The program assists guests with discharge planning focused on safety and wellness, and provides post-discharge health resource navigation support.
