About the Role
Ebenezer Square, a DePaul Community Residence-Single Room Occupancy (CR-SRO) Program, is seeking a mental health professional to provide direct support, advocacy, and education to residents. The role involves assisting residents with individualized service plans to enhance their autonomy, quality of life, and recovery process.
Responsibilities
- Collaborate with management to provide tours and lunch visits for prospective residents, offering input on their acceptance into the program.
- Ensure comprehensive, person-centered service planning for each resident, including recovery-oriented services and discharge planning, adhering to NYS Office of Mental Health standards.
- Conduct quarterly service plan reviews with residents and their support teams to assess progress and update plans.
- Provide teaching, monitoring, and recovery-oriented supports including Assertiveness/Self-Advocacy Training, Community Integration, Daily Living Skills Training, Health Services Education, Medication Management, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, and Symptom Management.
- Observe, monitor, document, and report resident behaviors and progress according to agency standards.
- Maintain accurate and timely resident files, including progress notes, service plans, and medical records.
- Schedule annual physical, eye, medical specialist, and dental appointments, arranging transportation as needed.
- Collaborate with Health Home Care Managers and community service providers for continuity of care.
- Act as a liaison between residents and clinical providers regarding mental health monitoring.
- Provide direct assistance with daily living skills, including room upkeep, meal preparation, and laundry.
- Assist residents in planning for necessary purchases like clothing and other essentials.
- Encourage healthy connections with family and communities, and help develop new supports and interests.
- Coordinate and participate in resident recreational activities.
- Assist Medication Coordinators with medication supervision and assume dining room responsibilities during breakfast when assigned.
- Provide safe transportation for residents when necessary.
- Maintain resident confidentiality and demonstrate a positive attitude towards residents, staff, guests, family, and service providers.
- Adhere to all safety rules and regulations.
- Attend required staff meetings and trainings.
- Act as a team member, providing input for treatment/support meetings and continued stay reviews.
- Perform other duties as necessary for program operation and resident well-being.
Requirements
- Must meet one of the following education and experience criteria:
- A. Bachelor's degree in Human Services field plus at least one (1) year of experience in a Human Services field.
- B. Associate's degree in a Human Services field plus at least three (3) years of related experience in a Human Services field.
- C. High School diploma/GED plus at least five (5) years of experience in a Human Services field.
- Must have a clean/valid New York State driving record.
- Must be at least eighteen (18) years of age.
