About the Role

By The Bay Health is seeking a Registered Nurse to serve as a Hospice Nurse Case Manager. This role is responsible for providing skilled nursing assessment, planning, and care to maximize patient and family comfort and health.

Responsibilities

  • Provide care in the home and work as a member of the interdisciplinary team comprised of nurses, social workers, chaplains, hospice aides and physicians.
  • Coordinate all patient/family services and prioritization of needs, including implementation of the individualized plan of care.

Requirements

  • One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
  • Experience in acute care/ home health and/or hospice setting preferred but not required.
  • Current license as a registered professional nurse in the State of California.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.
  • Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
  • Bi-lingual in Cantonese, Mandarin, or Taishanese required.

  • $65.00 - $75.70 hourly + language differential
