About the Role
UCP Long Island is seeking dedicated Licensed Practical Nurses (LPNs) for full-time or part-time shifts at its 31-bed intermediate care facility in Commack. The role involves assisting in the overall health and safety of individuals served, under the supervision of a Registered Nurse.
Responsibilities
- Administer medications and treatments as prescribed by physician.
- Generate incident, seizure, and illness reports.
- Monitor and report healthcare on daily flow report records.
- Instruct mid-level supervisors and direct care staff in hygiene, diet, weights, etc.
- Submit written monthly, quarterly, and annual nursing/medical progress notes.
- Coordinate laboratory care and medical appointments.
- Maintain interpersonal relationships with other disciplines.
- Maintain ongoing communication with other programs regarding client healthcare.
- Monitor program participants during mealtimes.
- Order, record, and maintain sufficient medication supplies.
- Maintain sufficient medical supplies and materials.
- Maintain accurate medical records, including medication administration sheets, Kardex, and Formulary.
- Provide emergency first aid.
- Coordinate emergency services.
- Supervise direct care staff in the absence of a mid-level supervisor.
Requirements
- Associate's degree or equivalent from a two-year college or technical school.
- One to six months of related experience and/or training, or an equivalent combination of education and experience.
- Must work at least one weekend shift per week on a recurring basis for all F/T and P/T LPN shifts.
Skills
- Knowledge of all medications for all individuals.
