About the Role
Person Centered Services is seeking a Project Manager to oversee organization-wide projects, serving as the central coordinator for prioritization, planning, execution, and monitoring to ensure initiatives align with organizational goals.
Responsibilities
- Manage the lifecycle of Executive Team-approved projects from initiation through completion.
- Lead project planning, implementation, tracking, and reporting activities.
- Facilitate the cross-functional project prioritization process.
- Coordinate project intake, impact assessments, resource planning, and implementation timelines.
- Maintain and monitor the organization's project portfolio and project prioritization framework.
- Lead cross-functional project teams and facilitate stakeholder collaboration.
- Identify and escalate project risks, barriers, and resource needs.
- Provide regular project status updates and reports to leadership.
- Support organizational change management and process improvement initiatives.
- Develop and maintain project documentation, timelines, and meeting records.
- Develop and maintain project governance tools, including project charters, RACI matrices, communication plans, and status reporting mechanisms to ensure clear accountability and stakeholder engagement.
- Perform other duties and special projects as assigned.

Requirements
- Bachelor's degree in Business, Healthcare Administration, Human Services, Project Management, or a related field.
- Three years of project management, operations, process improvement, or related experience.
- Master's degree in a related field (Preferred).
- PMP, Lean Six Sigma, or other project management certification (Preferred).
- Experience in healthcare, managed care, OPWDD, Health Home, or human services (Preferred).
Skills
- Strong project management and organizational skills.
- Ability to manage multiple priorities and competing deadlines.
- Excellent communication, facilitation, and relationship-building skills.
- Strong analytical and problem-solving abilities.
- Experience leading cross-functional teams.
- Proficiency with Microsoft Office, Teams, SharePoint, and project management tools.
- Familiarity with EMR systems, quality management, and operational workflows preferred.

- Flexible, open to all regions served
- Buffalo office (required work twice per week)
