About the Role
This role involves providing specialist Occupational Therapy assessments and interventions for adults in a community-based setting, aiming to maximize independence, improve wellbeing, and achieve personal goals.
Responsibilities
- Complete Occupational Therapy assessments for adults with a range of needs
- Assess functional ability and identify barriers to independence
- Develop person-centred intervention plans
- Recommend appropriate equipment and adaptations
- Complete risk assessments and support safe decision-making
- Promote independence and wellbeing through strengths-based practice
- Work collaboratively with multidisciplinary teams and partner agencies
- Support service users and carers to achieve agreed outcomes
- Maintain accurate and timely case records
Requirements
- HCPC registration as an Occupational Therapist
- Experience working within community/social care settings
- Strong assessment and clinical reasoning skills
- Ability to manage a caseload effectively
- Experience working with adults with complex needs
- Excellent communication and MDT working skills
- Confidence working independently within a community environment
