About the Role
Provide professional patient care to long-term and post-acute care residents. Direct and monitor the care provided by CNAs, ensuring proper documentation and recording. Lead CNAs and ensure quality patient care.
Responsibilities
- Administer medications and document care for assigned team.
- Perform and document treatments as per physician orders and facility protocol.
- Ensure concise, pertinent, and complete resident care documentation using a computerized medical record.
- Initiate appropriate care pathway tools.
- Direct CNAs in completing quality individualized personal care according to care plans.
- Ensure CNA documentation is accurate and complete.
- Receive and give relevant reports, both written and verbal.
- Contact physicians, transcribe orders, and follow up with appropriate action.
- Monitor residents and report relevant condition status to charge nurse/supervisor and/or physicians.
- Complete pre-reviews of medication administration records.
- Complete required MDS documentation and related observations.
- Assist CNA team by providing direct ADL care to residents.
Requirements
- Graduate from an accredited School of Practical Nursing.
- Current LPN licensure in State of New York.
- Basic Life Support (BLS) Certification from the American Heart Association (AHA).
- Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
