About the Role

This position will support the entire Continuum of Care team, including directors, Case Managers, Social Workers, and other members of the health care team in meeting discharge planning and continuity of care needs for patients and their caregivers. This position will support completing concurrent insurance reviews on the Rome Behavioral Health (RBHU) unit and managing referrals to skilled nursing facilities and home care agencies via the Care Portal.

Requirements

Basic knowledge of Case Management and Social Worker principles and practices
Knowledge of office terminology, procedures and equipment
Entry level of knowledge of medical terminology, business arithmetic, and English
Ability to prioritize and follow oral and written instructions
PC and appropriate software knowledge
