About the Role

Health Quest Home Care is seeking individuals to join their home healthcare team. This role involves working independently while collaborating with physicians and families to provide care in patients' homes. Responsibilities

• Assess physical, emotional, socio-economic, and environmental needs of patients and families.
• Provide physician-ordered services as per the plan of care and communicate with involved healthcare professionals.
• Develop, implement, and evaluate the plan of care in partnership with patients, caregivers, and physicians.
• Coordinate services with patients, families, and other involved parties, providing counseling and education.
• Regularly evaluate patient progress, take prompt action on condition changes, and terminate care when goals are met or per agency policy.
• Participate in agency quality assurance, performance improvement programs, and in-service training.
• Identify and refer patients and families for other appropriate home health services.
• Supervise homemaker/home health aide services according to guidelines and submit evaluations.
• Fulfill compliance responsibilities, including preparing clinical notes and attending in-service training.

Requirements

• RN diploma with over 3 years of job-related experience considered in lieu of an Associate's Degree.
• Current RN License in the State of New York.
• Strong research skills.
• Clear and concise interpersonal communication skills.
• Knowledge of systems integration.
• Minimum of one year of experience.
• Experienced in a qualified home health agency (Desired).
