About the Role
Support adults with intellectual, developmental, physical, and behavioral health needs in residential and community settings. Help individuals build life skills, achieve personal goals, and remain active in their communities as part of a supportive and mission-driven team.
Responsibilities
- Provide hands-on support for individuals with medical and light behavioral needs in a 14-bed medically frail group home.
- Assist adults with disabilities with daily living activities including cooking, cleaning, hygiene, dressing, mobility, and personal care.
- Support individuals with bathing, toileting, medication reminders, meal planning, budgeting, communication, and independent living skills.
- Provide transportation to medical appointments, errands, community outings, and recreational activities.
- Encourage participation in volunteer opportunities, fitness programs, Special Olympics, sporting events, crafts, and other community-based activities.
- Implement positive behavior reinforcement strategies and utilize de-escalation techniques.
- Assist with goal planning, progress tracking, socialization, and skill development to support individualized care plans.
- Collaborate with an interdisciplinary team including nurses, behaviorists, nutritionists, and other clinical professionals.
- Maintain accurate electronic and physical documentation of daily case notes, services provided, progress, and activities.
Requirements
- High school diploma or equivalent.
- Valid driver's license and reliable transportation.
- Willingness to learn with no prior experience required.
- Compassionate, dependable, and motivated to support others.
