About the Role
Visiting Nurse Services Westchester is seeking Physical Therapy professionals to join their team. Recent graduates are welcome to apply and will participate in a comprehensive home care training program. The company is known for its people-oriented culture and high employee retention rate.
Responsibilities
- Assess patients for functional limitations and develop a therapy plan of treatment with specific goals in collaboration with the patient, family, nurse, physician, and other health team members.
- Provide direct care to treat the medical/surgical condition through the use of accepted physical therapy modalities.
- Instruct and supervise the patient/family and home health aide in the home exercise program and other aspects of patient care.
- Document patient response to instruction.
- Continuously re-evaluate the patient’s condition and modify the plan of treatment as appropriate.
- Identify and report significant clinical information to the physician, clinical manager, primary nurse, and other disciplines.
- Document all clinical evaluations, treatments, and patients' response to therapy.
Requirements
- NYS professional license and current registration as a Physical Therapist.
- One (1) year of full-time experience in direct provision of physical therapy service.
- Ability to assess and respond to the needs of patients and families in the home setting.
- Ability to utilize industry technology such as a tablet when in the field with patients.
- Ability to drive with automobile in good working condition and available for use in the field.
Skills
- Home care experience preferred.
- Experience with Oasis assessments is preferred.

- $100,000 to $116,000 annually
