About the Role
Seeking an experienced Primary Care Nurse Practitioner (NP) for two outpatient Family Medicine practices serving pediatric and adult patients. This full-time hybrid role involves two days of patient-facing care and three days of remote work, including telehealth visits and in-basket coverage. The position offers outstanding clinical and administrative support in a collaborative primary care environment.
Responsibilities
- Provide patient-facing care in clinic two days per week.
- Conduct telehealth visits three days per week.
- Manage acute and chronic conditions across the lifespan.
- Perform preventive care visits.
- Order and interpret diagnostic testing.
- Develop treatment plans.
- Prescribe medications within scope of practice.
- Document care in the electronic medical record.
- Communicate effectively with patients, families, physicians, and the care team.
- Provide telehealth visits supporting both Clinton Street and Windsor offices.
- Offer flexibility in telehealth hours based on patient and practice needs.
Requirements
- Board certified.
- Eligible for licensure in the State of New York.
- Prior experience as a Nurse Practitioner in family medicine, primary care, pediatrics, internal medicine, or a related outpatient setting.
- Not open to Physician Assistants or new graduate applicants.
- Three (3) years of professional nursing experience in a clinical setting.

Salary/Compensations
- $131,900 - $155,200
