About the Role

Seeking an experienced Primary Care Nurse Practitioner (NP) for two outpatient Family Medicine practices serving pediatric and adult patients. This full-time hybrid role involves two days of patient-facing care and three days of remote work, including telehealth visits and in-basket coverage. The position offers outstanding clinical and administrative support in a collaborative primary care environment.

Responsibilities

• Provide patient-facing care in clinic two days per week.
• Conduct telehealth visits three days per week.
• Manage acute and chronic conditions across the lifespan.
• Perform preventive care visits.
• Order and interpret diagnostic testing.
• Develop treatment plans.
• Prescribe medications within scope of practice.
• Document care in the electronic medical record.
• Communicate effectively with patients, families, physicians, and the care team.
• Provide telehealth visits supporting both Clinton Street and Windsor offices.
• Offer flexibility in telehealth hours based on patient and practice needs.

Requirements

• Board certified.
• Eligible for licensure in the State of New York.
• Prior experience as a Nurse Practitioner in family medicine, primary care, pediatrics, internal medicine, or a related outpatient setting.
• Not open to Physician Assistants or new graduate applicants.
• Three (3) years of professional nursing experience in a clinical setting.
