About the Role
Utilizes various activities under supervision to serve the treatment needs of clients and help them make progress toward their goals.
Responsibilities
- Conduct required assessments for new clients and on an ongoing basis with existing clients
- Develop treatment plans in collaboration with recipients; review and update as required
- Meet individually with clients to address progress/barriers toward treatment goals
- Maintain contact with collaterals to coordinate care/treatment of clients
- Coordinate discharge plans once the client is ready to move on from program
- Facilitate multiple therapeutic and skill-building groups throughout the day
- Utilize evidence-based materials when facilitating groups
- Follow documentation and billing guidelines for all individual and group services; review all documentation/billing to ensure for accuracy.
- Work as part of a multi-disciplinary team in developing and executing interventions for optimal client care
- Represents program at community meetings on an as-needed basis
- Ensures a safe, secure and nurturing environment is provided and maintained for clients
- Identify and implement quality improvement trainings and activities as part of professional development goals
- Complete QI Training during onboarding and participate in QI refresher training as needed.
- Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
- Adherence to all safety protocols and procedures to ensure a safe working environment for all employees.
- Demonstrate a commitment to maintaining a safe work environment by following established safety guidelines.
- Include any relevant knowledge or experience regarding health and safety protocols to ensure a safe work environment for all employees.
- Other duties as assigned.
Requirements
- Experience working with adults diagnosed with mental illness and facilitating groups preferred.

Education Level
- Associates Degree required
- Bachelor's degree or higher preferred.
