About the Role

The majority of our patients prefer to receive hospice and palliative care at home to maintain their independence and live their lives to the best of their ability. Symptoms are managed so that the patient can live a full life with the support of family and friends and the comforts of home. A Home Care Licensed Practical Nurse (LPN) is responsible for carrying out patient care in accordance with physician's orders under the guidance and direction of the Registered Nurse in charge and supervision of the Team Manager.

Responsibilities

Provides skilled nursing care to patients as directed by the IDG plan of care in accordance with agency policies, including personal care, vital signs, and administration of medications, treatments, and procedures within their professional practice.
Documents to agency standards, timely completion, timely signatures, using A-I-R protocol.
Reinforces patient and family teaching regarding disease process, self-care techniques, and prevention strategies under the guidance of the assigned registered nurse.
Assists in planning and exchanging information with other health professionals to achieve and maintain continuity of patient care.
Assists the physician and registered nurse in performing specialized procedures.
Elicits patient/family complaints or unusual occurrences and reports reportable incidents.
Demonstrates proper sharps disposal, hand hygiene, waste disposal, and use of personal protective equipment.
Communicates current health status and needs of patients to appropriate health professionals, contributing to the coordination of the total plan of care.
Participates in the quality improvement program and documents quality assurance data as required.
Participates in weekend, holiday rotation, and back-up visits as needed.
Adheres to organizational policies and procedures, both patient-focused (clinical) and organizational-focused (administrative).
Adheres to HIPPA regulations and policies and maintains the security of patient electronic/written records.

Requirements

Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.
