Responsibilities
- Provide comprehensive primary care services to nursing home residents.
- Perform new admission evaluations and ongoing follow-up visits.
- Monitor, assess, and manage acute and chronic medical conditions.

- Document patient encounters thoroughly and accurately in the medical record.

Requirements
- Active Nurse Practitioner license in the state of New York.
- Active DEA license.
- Minimum 6 months of experience working as a Nurse Practitioner.
- Prior nursing home or long-term care experience strongly preferred.
