About the Role

VHA Home HealthCare is seeking four Registered Practical Nurses (RPNs) to join their Hospital Partnership Program. This program focuses on providing care to clients for 90-120 days following hospital discharge, ensuring safe, personalized care in the client's home. Nurses will conduct comprehensive assessments and deliver various services, coordinating care with hospital partners.

Responsibilities

  • Assess, monitor, and make care planning and care coordination.
  • Evaluate care plan interventions through on-going reassessment and evaluation results.
  • Ensure the quality of patient care by identifying and acting on factors which affects practice autonomy and delivery of quality care.
  • Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
  • Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
  • Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan, and submit appropriate documentation to the case coordinator.
  • Maintain documentation standards according to the College of Nurses of Ontario.
  • Participate in a standby/on-call rotation to be available for answering calls and respond, if needed.
  • Participate in team meetings, as required during the stated availability.

Requirements

  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • RPN diploma from a recognized university or college.
  • Valid First Aid or CPR Certification.
  • A valid Ontario driver’s license (G2 or G License) and a reliable vehicle for traveling are required.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.

Work Type

  • Full-time
  • Day shifts (09:00-13:00 or 09:00-17:00)
  • Evening shifts (15:00-23:00)
  • Bi-weekly weekend rotation

Experience Level

  • Acute Care and Medical-Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.

Education Level

  • RPN diploma from a recognized university or college.

Salary/Compensations

  • $26.60 - $32.54 /visit (placement on salary range as per Collective Agreement) + $5.00 Top Up Premium Per Visit
  • Estimated annual earning potential: $70,000–$120,000. Actual earnings may vary based on availability and service volumes
