About the Role

The London HART Hub is a low-barrier, recovery-oriented space supporting individuals experiencing homelessness with mental health and addiction needs. Counsellors develop therapeutic relationships, provide addiction treatment planning, increase client motivation, and offer counselling to support client wellness and improve their bio-psycho-social-spiritual health.

Responsibilities

• Provide Intake Services, including screening and interpretation of assessment tools.
• Recognize and respond effectively to individuals/groups/communities in crisis.
• Provide crisis intervention and de-escalation.
• Adapt services to clients’ social and cultural needs and provide accessible communication options.
• Assess client transportation needs and provide assistance.
• Utilize standardized provincial assessment tools and adhere to quality assurance measures.
• Provide clinical interpretation of assessment tools and utilize motivational interviewing.
• Develop client-centred treatment plans and re-assess on an ongoing basis.
• Develop and maintain therapeutic relationships with clients.
• Increase client motivation using a Stages of Change paradigm and motivational interviewing.
• Apply evidence-informed counselling styles and techniques.
• Use best practice, promising practices, and evidence-informed models in individual or group settings.
• Develop and update formalized treatment plans, including case management, goal setting, counselling, referrals, advocacy, and education.
• Match clients with appropriate services through screening, assessment, referral, and treatment planning.
• Manage and meet targets for a clinical caseload and maintain direct service requirements.
• Participate in the facilitation of community treatment groups.
• Facilitate successful service navigation for clients through empowerment and/or advocacy.
• Participate and lead client-centred case conferences.
• Coordinate services for individuals involved in the social justice system.
• Provide support regarding addiction treatment, including understanding of addiction types, treatment continuum, and continuum of use.
• Provide rapid connection to housing stability resources.
• Provide services in person, by phone, or other communication means regarding addiction and mental health services.
• Provide education to clients and concerned others regarding coping techniques and strategies.
• Deliver effective outreach services targeting prevention, health promotion, early intervention, treatment, and/or after care.
• Engage in community development, community awareness, education, and outreach.

Requirements

• College Diploma or University Degree in a related field (e.g., BSW, Post-graduate diploma in addiction) or equivalent.
• Currently registered and in good standing with a professional college (e.g., OCSWSSW, CNO, CRPO, CPSO, COTO, CPO).
• Valid Driver’s License: minimum G2 class required.
• Transportation of clients in personal vehicle required; two million-dollar ($2,000,000.00) liability insurance required.
• Current, satisfactory Police Vulnerable Sector Check (PVSC).
• Demonstrated ability to attend work regularly.

Salary/Compensations

• Starting rate: $30.01 per hour for Level 1
• Grid progression up to $37.26 per hour for regulated staff
• Premiums of $2.00 per hour for eligible Evening/Night and Weekend Shifts
