About the Role

Family Service League is seeking a part-time hourly Psychiatric Nurse Practitioner for the Assertive Community Treatment (ACT) program. This role involves assessing and diagnosing clients' mental health, developing care plans, and providing ongoing community-based care including home visits, vital sign checks, and medication administration. Travel is required.

Responsibilities

  • Prescribe and revise medical regimens according to collaborative practice agreements and protocols.
  • Administer and monitor medications, including injectable medications.
  • Provide medical assessments and vitals for consumers in the community.
  • Conduct health assessments of adults with potential acute and/or chronic health problems.
  • Initiate referrals to specialty services and foster continuity of care.
  • Initiate selected therapeutic and diagnostic procedures, and consultation and laboratory examinations.
  • Provide patient/family education and counseling in health promotion, maintenance, and disease prevention.
  • Participate in team meetings and conferences for an interdisciplinary approach to primary health care.
  • Serve as liaison between patient and community, and act as a health care advocate.
  • Identify quality improvement issues and document standards of care and practice.
  • Maintain a current level of knowledge relative to professional practice.
  • Act as a resource to staff and implement educational programs.
  • Participate in the evaluation of the cost effectiveness of services provided.
  • Perform all other duties as assigned.

Requirements

  • Nurse Practitioner degree and license required.
  • State licensure and DEA credentials required.
  • A minimum of 3 years of prior prescribing experience is required.
  • Proficient computer skills, including Microsoft Office required.
  • Experience with an EMR is preferred.
  • Excellent time management, interpersonal, and verbal and written communication skills required.
  • Ability to work with a multi-disciplinary team in providing treatment to persistent, severe mentally ill population required.
  • Valid and clean NYS Driver’s License required.
  • This position requires sitting for long periods of time and travelling to conduct home visits.
