About the Role

RiseWell Community Services is seeking a Full-Time Team Leader for the SOS Team in Brooklyn, NY, to implement and maintain the program model, oversee daily operations, and support the team in community outreach and participant coordination.

Responsibilities

• Implement and maintain the program model.
• Oversee daily operations of the critical support team and program.
• Assist the team with community outreach on streets and subways.
• Coordinate participant needs before and after moves, including daily living skill enhancement, appointment accompaniment, and advocacy.
• Develop and maintain collaborative relationships with community partners such as the Department of Homeless Services, MTA, local law enforcement, community hospitals, outreach teams, and housing providers.

Requirements

• Master’s degree or higher in Social Work, Mental Health Counseling, Public Administration, Nursing, Public Health, Public Policy or related field.
• Minimum of 3 to 5 years post-masters work experience in the behavioral health and criminal justice field.
• Experience with target population preferred.
• Previous management experience preferred.
• Knowledge of NYC subway system.
• Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
