About the Role

Work as a member of the Overnight multi-disciplinary team to assess the health and social needs of the homeless population, including late evening to early morning hours and weekends.

Responsibilities

• Assist the team with community outreach and canvassing on streets and subways.
• Coordinate participant referrals with appropriate overnight facilities.
• Provide accompaniment, advocacy, and assistance to ensure warm handoffs and follow-ups with SOS day teams.
• Maintain collaborative relationships with community partners including the Department of Homeless Services, MTA, local law enforcement, community hospitals, outreach teams, and housing drop-in shelters.

Requirements

• A minimum of 4 years of experience in case management.
• Bachelor’s degree in Human Services preferred with previous case management experience.
• Clean valid NY State Driver’s License.
• Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.
