About the Role
Suncrest Hospice aims to redefine hospice care by offering exceptional service and support to patients and their families. We achieve this by ensuring adequate staffing for increased clinical visits and reduced clinician caseloads. As a CHAP-certified hospice, we seek dedicated individuals committed to high-quality patient care.
Responsibilities
- Provide resident care to assigned residents in accordance with resident assessments and care plans.
- Collaborate with licensed care staff to coordinate resident care.
- Adhere strictly to Company nursing policies, procedures, and safety guidelines.
Requirements
- Strong commitment to improving quality of life through patient care partnerships with community organizations.
- Ability to collaborate effectively within a team environment with nurses, social workers, chaplains, volunteers, and STNA leadership.
- Willingness to drive with reliable transportation, a valid driver's license, and auto insurance.
- Certified Nursing Assistant (CNA) certification in the state(s) of practice.
- Home Health Aide (HHA) certification in the state(s) of practice.
- CPR certified.
- CNADC certified.
