About the Role

The Adult Health Home Care Manager (HHCM) will assist adults eligible for and in need of care coordination. The Care Manager will identify and address physical, behavioral, and social health needs, establishing an integrated care team to reduce emergency room visits, hospitalizations, and out-of-home placements.

Responsibilities

• Work with adults to determine eligibility for Health Home Care Management services and assist with enrollment.
• Conduct ongoing assessments to identify strengths and needs, developing comprehensive care plans with clear goals for health and wellness.
• Coordinate care and collaborate with multidisciplinary team members to ensure quality care.
• Refer individuals to healthcare providers, mental health and substance use providers, medications, housing, social services, and other community resources.
• Conduct home and community-based visits to engage members.
• Manage an average caseload of 30 clients.
• Participate in the on-call rotation for after-hours emergency/crisis situations.

Requirements

• Minimum of a bachelor’s degree with 2 years of relevant experience in health or human services; or master’s degree with 1 year of relevant experience; or Registered Nurse with 2 years of relevant experience.
• Previous care coordination/case management experience working with adults living with Serious Mental Illness, developmental disabilities, substance use disorders, or chronic medical conditions preferred.
• Must have a valid New York State driver’s license and access to reliable transportation.
