About the Role

Optum Home & Community Care is creating a new integrated care model to holistically address an individual’s physical, mental, and social needs. The Telehealth Urgent Care program provides telephonic/telehealth care and direction to patients, caregivers, and facility staff, offering 24/7 coverage. This remote role allows for virtual care delivery, fostering work-life balance and offering significant career development opportunities within Optum.

Responsibilities

  • Provide telephonic/telehealth care and direction to patients, caregivers, and facility staff.
  • Coordinate and manage care for members by taking and placing calls between caregivers, facilities, hospitals, primary care providers, and Optum field colleagues.
  • Utilize video platform based on clinical need.
  • Perform comprehensive assessments and document findings in a concise and comprehensive manner compliant with documentation requirements and CMS regulations.
  • Ensure encounter documentation supports the diagnosis.
  • Ensure all quality elements are addressed and documented.
  • Utilize evidenced-based practice guidelines.
  • Attend and complete all mandatory educational and MyLearning training requirements.
  • Coordinate care as members transition through different levels of care and care settings.
  • Monitor member and family needs and facilitate adjustments to the plan of care.
  • Review orders and interventions for appropriateness and response to treatment.
  • Conduct advanced care plan conversations with members and families.
  • Evaluate the plan of care for cost-effectiveness.
  • Provide excellent customer service to members/families, the interdisciplinary care team, and Optum staff.
  • Communicate effectively with internal and external parties including physicians, patients, key decision-makers, nursing facilities, field staff, and other provider groups.
  • Meet shift scheduling requirements and attendance expectations.
  • Exhibit original thinking and creativity in developing new and improved methods.
  • Function independently and responsibly with minimal supervision.
  • Demonstrate initiative in achieving individual, team, and organizational goals.
  • Participate in quality initiatives.
  • Check Optum email intermittently for required trainings, communications, and monthly scheduling.

Requirements

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Active and unrestricted license in the state of residence and in CT, NY, NH, ME, RI, NJ; ability to obtain licenses in other required locations.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire per state regulations.
  • Active Nurse Practitioner certification through a national board.
  • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner.
  • Ability to gain a collaborative practice agreement, if applicable in your state.
  • If practicing autonomously or without formal supervision, must have obtained approval from the licensing board or begin working towards it within 1 month of hire and apply within 3 months of becoming eligible.
  • Must adhere to UnitedHealth Group’s Telecommuter Policy.
  • Ability to work scheduled shifts in accordance with scheduling policies.
  • Proficient computer skills including the ability to document medical information with written and electronic medical records.
  • Candidates are required to pass a drug test before beginning employment.
