About the Role
Under general supervision, provides services, support, and treatment to clients. Maintains confidentiality and ethical standards. Supports ongoing communication amongst program staff and ensures a trauma-informed care environment.
Responsibilities
- Provides services to clients and their families primarily through face-to-face interactions.
- Services include counseling, training/education, advocacy, home studies, life skills, and documentation.
- Completes assessments, formulates, implements, and monitors service delivery plans.
- Coordinates service delivery and completes required evaluations and screenings (medical, educational, legal, law enforcement, vocational, mental health).
- Facilitates and encourages connection with appropriate cultural and community resources.
- Attends appointments, including those required by Courts, and may provide transportation.
- Works collaboratively with Monroe County Department of Human Services and other service providers.
- Assists clients with crises such as lack of food, housing, abuse, or domestic violence.
- May represent the program at county-level client service plan meetings.
- Attends and participates in local community events.
- Maintains knowledge of community resources available to the client population.
- Meets regularly with supervisor to evaluate and implement professional growth.
- Informs supervisor in a timely fashion about significant client-related events.
- Participates in required Agency/Department/Program meetings, trainings, and program development.
- Initiates professional development through attendance at outside conferences, in-service training, and reading relevant literature.
- Completes required record documentation in accordance with Program Policies and Procedures.
- Maintains documented client case records including all significant client interventions.
- Ensures timely preparation and submission of required reports and statistics.
- Completes required record documentation in accordance with the program’s policies and procedures.
- Participates in relevant Agency meetings and/or trainings.
- Actively pursues development of professional competencies through reading, supervision, in-service training, and attendance at conferences and workshops.
- Demonstrates awareness and active support of the Agency's Mission, Vision, Values, and Strategic Plan.
- Performs other duties as assigned.
Requirements
- Bachelor’s Degree in Social Work, Child Welfare or similar experience preferred.
- Equivalent combination of education and experience will be considered.
- Valid and clean NYS Driver’s License.
- Reliable transportation.
- Two or more years of experience in the child welfare or community service fields preferred.
- Ability to prioritize assignments, plan, and complete work projects with minimal direction.
- Ability to work efficiently and effectively and meet deadlines.
- Ability to work under pressure.
- Excellent verbal/written skills.
- Ability to maintain confidentiality.
- Ability to demonstrate behaviors and attitudes that support agency mission, philosophy, and policies.
- Willingness and ability to foster agency and program-wide cooperation and teamwork through positive/constructive communication techniques.
- Possession of a valid NYS Driver’s license and use of a registered and reliable vehicle.
- Employee is frequently required to sit, talk, hear, stand, walk, use hands to type and/or perform light lifting.
- Specific vision abilities required include close vision, distance vision and ability to adjust focus.
- Adheres to all applicable federal and state laws and regulations including those governing confidentiality, privacy, program, billing and documentation standards.
