About the Role
Seeking a detail-oriented and compassionate Licensed Practical Nurse (LPN) to ensure the accuracy and timeliness of resident assessments, supporting quality care, regulatory compliance, and appropriate reimbursement. This role is essential in developing individualized care plans that promote the best possible outcomes for residents.
Responsibilities
- Coordinate and complete MDS assessments accurately and within required timeframes.
- Ensure compliance with federal and New York State Department of Health regulations.
- Collaborate with the interdisciplinary team to develop and update individualized care plans.
- Monitor documentation to ensure accuracy, completeness, and regulatory compliance.
- Support quality improvement initiatives related to resident assessments and outcomes.
- Assist in preparation for regulatory surveys and audits.
- Maintain confidentiality of resident information and documentation.
- Communicate effectively with residents, families, providers, and interdisciplinary team members.
- Prioritize multiple responsibilities while meeting established deadlines.
Requirements
- Graduate of an accredited Licensed Practical Nursing (LPN) program.
- Current New York State Licensed Practical Nurse (LPN) license.
- Minimum of three (3) years of experience as an LPN in a long-term care setting.
- Ability to maintain confidentiality and exercise sound judgment.
- Strong organizational, communication, and time management skills.
- Demonstrated ability to manage multiple priorities and work collaboratively as part of a team.
- Computer proficiency.
- BLS certification through RQI upon hire.
Skills
- Knowledge of the MDS assessment and care planning process.
- Familiarity with New York State Department of Health regulations and survey process.
- Experience using MDS/care planning software.
- Proficiency with Microsoft Office applications.
Experience Level
- 3 years of experience as an LPN in a long-term care setting
Education Level
- Licensed Practical Nursing (LPN) program graduate
