About the Role

Care Coordinators link adults and children with chronic behavioral health and medical conditions to the services they need to stay as healthy as possible and inspire members to use those services to optimize their health outcomes. Working in a team setting and primarily in the field, Care Coordinators assess risk and needs, develop person-centered care plans, provide care management services, track and arrange appointments, educate members, and coordinate other aspects of members’ health and community services.

Responsibilities

  • Integrate medical, specialized, and behavioral health services, along with social support and/or educational support services.
  • Periodically assess a member’s medical and behavioral health needs and compliance with recommended treatments.
  • Collaboratively develop an Individualized Care Plan (ICP) with the member, their family/caregivers, and other service providers.
  • Provide required care management services.
  • Track all specialty medical, behavioral, and support service referrals using provided Health Information Technology (HIT).
  • Ensure members have access to, engage in, and retain needed services as defined in the ICP, including acute/primary/preventative medical care, home health care, chemical dependency services, behavioral health services, community social support, housing, state/federal entitlements, educational services, and involvement with justice or child welfare systems.
  • Provide outreach services to members to increase access to services.
  • Respond to members’ information and referral questions.
  • Reassess the need for ongoing care coordination services.
  • Complete all required documentation.
  • Share knowledge and experience with team members to support overall service provision.
  • Carry an agency-provided cell phone.
  • Respond to member crises during and occasionally outside of regular business hours.
  • Perform other duties as assigned.

  • Ability to travel within New York City carrying equipment weighing up to approximately 10 pounds.
  • Ability to routinely sit (60% of the time), stand (20% of the time), and travel to and from appointments using varied public and private transportation options (20% of the time).

Experience Level

  • Two years of experience in providing direct services, or a substantial number of case management services, to mentally disabled or chronically ill or homeless individuals, or children with complex social or healthcare needs (if applicable with specific degree).
  • Five years’ experience working with an applicable population (if applicable with a different degree).

Education Level

  • Bachelor's degree with a major or concentration (minimum of 24 credits) in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing.
  • NYS teacher's certification for which a bachelor's degree is required.
  • NYS licensure and registration as a Registered Nurse and a bachelor's degree.
  • Bachelor’s Degree, Associates Degree or High School Diploma/GED in another discipline.
