About the Role
NYCCC - Queens Campus is seeking services of a valid licensed Speech-Language Pathologist for children/adolescents. Services to be provided shall include patient evaluations, treatment planning, speech therapy, and consultation to staff.
Responsibilities
- Provide patient evaluations, treatment planning, speech therapy, and consultation to staff.
- Document evaluations and therapy per Uniform Case Record and NYCCC policies.
- Provide services not exceeding 20 hours per week, with a maximum of 1040 hours annually.
- Adhere to all requirements, regulations, ethical practices, standards, and licensing requirements.
- Complete HIPAA/HITECH and other mandated training.
- Wear facility-issued badge and identification at all times.
- Integrate into the treatment team and interact professionally with staff at all levels.
Requirements
- Must possess a valid license to practice in the State of New York.
- Must provide updates to license upon facility request.
- Must be a NYS Teacher of Speech and Hearing Handicapped.
- Must have direct clinical experience working with children/adolescents.
- Must provide resumes and mandatory forms prior to interviewing.
- Must provide proof of immunizations, including PPD/Tuberculosis testing and flu vaccine.
- Must complete the first four of five mandatory forms for clearance prior to interview.
- Must complete the Health Clearance Form.
- Must be certified as a TSSH or TSSLD for the State of New York.
