About the Role

The Clinical Case Manager provides case management and clinical services to BSLP clients, with an emphasis on therapy, crisis intervention, treatment planning, and psychosocial assessments for adults facing mental health, substance use disorder, and transitional housing challenges within the PRC Baker Supportive Living Program.

Responsibilities

• Provide individual behavioral health counseling and case management to support clients with mental health and/or substance use disorder needs.
• Offer group therapy and rehabilitation services for clients with mental health and/or substance use disorder needs.
• Coordinate with external providers to ensure clients access necessary medical, mental health, substance use disorder, and other support services.
• Conduct outreach to clients in their residences, providing on-site services to those who may not otherwise access care.
• Perform initial and updated psychosocial and ASAM assessments.
• Develop and adjust individualized treatment plans.
• Ensure timely and agency-compliant documentation.
• Deliver culturally competent services utilizing harm reduction and trauma-informed principles.
• Respond to mental health crises, providing immediate services, de-escalation, and follow-up care.
• Participate in rotating on-call duties for after-hours crisis response.
• Maintain a target productivity level based on contractual requirements.
• Manage a caseload of at least 25 clients, adjusting as needed based on occupancy and client requirements.
• If licensed, maintain 51/50 certification and respond to assessments and follow-ups as required.
• Provide step-down case management for clients transitioning to specific housing arrangements.
• Promote the agency's mission, purpose, and values in all professional interactions.
• Maintain licensure or license-eligible status, complete required annual training, and maintain ANSA and ASAM certifications.
• Maintain CPR and First Aid certifications.
• Comply with annual TB testing requirements.
• Perform additional duties as assigned.

Requirements

• Actively Licensed or registered with the California Board of Behavioral Sciences (BBS) as an LCSW, LMFT, Psy.D., ASW, AMFT, LPCC, or APCC.
• Possess two years of clinical experience working with adults with a history of mental health issues.
• Possess two years of clinical experience working with adults with a history of substance use disorder.
• Possess knowledge of San Francisco community resources serving the target population.
• Demonstrate a strong practical and theoretical foundation in clinical MH/SUD evidence-based best practices.
• Exhibit exceptional verbal and written communication skills.
• Proficient in electronic medical record systems and Microsoft Office products.
• Willingness to work non-traditional hours as required by the job.
• Maintain First Aid and CPR Certifications.
• Effective time management skills and ability to meet deadlines.
• Availability to work flexible hours outside of assigned shifts occasionally.
• Exhibit exemplary customer service, compassion, and care in all duties and interactions.
