About the Role

The Clinical Case Manager provides essential case management and clinical services to AILP clients, focusing on therapy, crisis intervention, treatment planning, and psychosocial assessments for adults facing mental health, substance use disorder, and transitional housing challenges.

Responsibilities

• Provide individual behavioral health counseling and case management for clients with mental health and/or substance use disorder needs.
• Facilitate group therapy and rehabilitation sessions for clients with mental health and/or substance use disorder needs.
• Coordinate with external providers to ensure residents access necessary medical, mental health, substance use disorder, and other support services.
• Conduct outreach to clients by traveling to their residences and providing on-site services.
• Perform initial and updated psychosocial assessments.
• Develop and adjust individualized treatment plans.
• Ensure timely and accurate documentation meeting agency standards.
• Deliver culturally competent services utilizing harm reduction and trauma-informed principles.
• Respond to mental health crises, providing immediate access to services, de-escalation, and follow-up care.
• Participate in rotating on-call duties for crisis response outside of business hours.
• Meet productivity targets based on contractual requirements.
• Maintain a caseload of at least 25 clients.
• If licensed, complete and maintain 51/50 certification and respond as needed for assessment and follow-up.
• Provide step-down case management for clients transitioning to specific housing arrangements.
• Promote agency mission, purpose, and values in all interactions.
• Maintain licensure or license-eligible status, complete required annual trainings, and maintain ANSA and ASAM certification.
• Maintain CPR and First Aid certifications.
• Comply with annual TB testing requirements.
• Perform additional duties as assigned.

Requirements

• Actively Licensed or registered with the California Board of Behavioral Sciences (BBS) as an LCSW, LMFT, Psy.D., ASW, AMFT, LPCC, or APCC.
• Possess two years of clinical experience working with adults with a history of mental health issues.
• Possess two years of clinical experience working with adults with a history of substance use disorder.
• Possess two years of clinical experience working with adults with a history of homelessness (preferred).
• Knowledgeable about San Francisco community resources serving the target population (preferred).
• Strong practical and theoretical foundation in clinical MH/SUD evidence-based best practices.
• Exceptional verbal and written communication skills.
• Proficiency in electronic medical record systems and Microsoft Office products.
• Willingness to work non-traditional hours as required by the job.
• Maintain First Aid and CPR Certifications.
• Effective time management skills and ability to meet deadlines.
• Availability to work flexible hours outside of assigned shifts occasionally.
• Exhibit exemplary customer service, compassion, and care in all duties and interactions.
