About the Role

This role is part of the multidisciplinary comprehensive breast survivorship team, focusing on patient identification, tracking, and providing holistic care summaries and follow-up plans. The position requires excellent communication, independent judgment, and a compassionate approach to patient care.

Responsibilities

  • Identify and track appropriate patients.
  • Review cancer care treatment summaries and follow-up care plans with patients and families.
  • Provide a comprehensive, holistic summary of individualized resources to help survivors.
  • Provide sexual wellness care.
  • Triage phone calls and manage urgent add-on clinic visits.
  • Conduct scheduled ambulatory long-term follow-up clinics.
  • Demonstrate clinical competence, superior verbal and written communication skills, interpersonal effectiveness, and customer relation skills.
  • Exhibit computer competence and an active desire to learn.
  • Participate in the education of learners (students and new employees) through clinical learning experiences, knowledge sharing, and skill training.

Requirements

  • Excellent communication skills with proven ability in exercising independent judgment and initiative as an advanced practice provider.
  • Flexibility with the growing needs of the practice and dependability.
  • Compassionate demeanor with a deep passion and interest for this patient population.
  • Self-directed with strong assessment and communication skills.
  • Ability to multi-task.
  • Prefer prior hematology/oncology or survivorship experience as an advanced practice provider.
