About the Role

The Nurse Practitioner will report to the Medical Director or supervisory Attending Physician and is responsible for delivering compassionate and quality medical care to assigned residents in coordination with the healthcare team.

Responsibilities

  • Examine residents and deliver quality care in varied settings including subacute, long term, rehab, and outpatient care.
  • Assist in resident/patient/family education.
  • Re-examine, re-evaluate, and modify care plans as necessary, including maintaining proper documentation in resident charts.
  • Review all diagnostic testing results and make recommendations.
  • Educate and supervise rotating medical and physician assistant students.

Requirements

  • Graduate of an accredited nurse practitioner program.
  • Active license as an NP in New York State, with prescriptive privileges.
  • NYS Board Certified.
  • Proficient in SigmaCare or another EMR (Electronic Medical Record) software.
  • Prior experience with the geriatric population preferred.
  • Two (2) or more years of experience in a sub-acute or long term care facility.
