Assist Unit Leadership in assessing patient functioning, and in developing and implementing treatment plans and services as patient needs warrant.

Provides direct care services to patients individually and in groups, including activities of daily living, social, and psychiatric rehabilitation per treatment plan, as directed by Unit Leadership.
Participates in the development of a comprehensive treatment plan, sharing patient needs and wants as communicated by the patient through surveys, collaterals, and the treatment team.
Documents and communicates patient response to treatment effectively, accurately, and in a timely manner according to Program Policies and Procedures.
Provides comprehensive care to patients in a professional and caring manner, taking appropriate actions and communicating findings to the multidisciplinary team.
Intercedes on behalf of customers to ensure they have access to required services.
Participates in the peer review process and utilizes quality improvement techniques to improve quality and reduce costs.
Participates as a member of the multidisciplinary team by providing feedback, maintaining professional relationships, and offering program and patient-specific feedback and recommendations.
Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
Understands and practices established RGHS policies, procedures, goals, standards of care, quality improvement activities, safety, and environmental and infection control.

Minimum of 2 years of experience in a Human Services related field, medical related field, customer service, or client/patient interfacing experience required.
Associate’s degree required.
Bachelor’s degree preferred.
