About the Role

Optum Home & Community Care is creating an integrated care model to holistically address an individual’s physical, mental, and social needs, helping patients access and navigate care anytime and anywhere. As a team member, you will help patients navigate the health care system and connect them to key support services, enabling them to stay well at home. This role offers the ability to achieve work-life balance and provides opportunities for career development.

Responsibilities

Provide telephonic/telehealth care and direction to patients, caregivers, and facility staff.
Provide virtual care for patients in various settings.
Achieve work-life balance.
Work a minimum of 4 weekend shifts per month (32 hours in a four-week period).
Provide additional availability of a minimum of 12 hours per month.
Provide holiday coverage from 5 PM the day before a holiday to 8 AM the day after a holiday.
Provide coverage for one 8-hour summer holiday (Memorial Day, Labor Day, or July 4th).
Provide coverage for one 8-hour winter holiday (Thanksgiving, Day After Thanksgiving, Christmas, or NYD).

Requirements

Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
Active and unrestricted license in the state in which you reside, as well as New York and Massachusetts, and ability to obtain in other required locations.
Ability to gain a collaborative practice agreement, if applicable in your state.
APCs working in jurisdictions that authorize autonomous practice must have obtained approval or be working towards it within specified timelines.
Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice).
Ability to work scheduled shifts in accordance with scheduling policies.
Proficient computer skills including the ability to document medical information with written and electronic medical records.
Driver’s License and access to reliable transportation.
3+ years of clinical experience as an APC (Preferred).
Active and unrestricted license in additional states (Preferred).
Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs (Preferred).
Experience working in a nursing home, or with seniors in an acute care facility (Preferred).
Understanding of Geriatrics, Chronic Illness, and acute disease management (Preferred).
Understanding of Advanced Illness and end of life discussions (Preferred).
Ability to develop and maintain positive customer relationships (Preferred).
Adaptability to change (Preferred).
Pass a drug test before beginning employment.
