About the Role
The LPN will work with clinical and clerical support staff to deliver excellent patient care and promote patient satisfaction across multiple practices. The LPN will practice under the direction of the appropriate clinical staff and cross cover across multiple practices.
Responsibilities
- Monitor patient's vital signs, patient intake and output, and recording patient history.
- Administer medication, injections and functions in scope of license.
- Clean equipment in patient care areas and ensure patient care rooms are kept clean, and supplies are well stocked.
- Maintain records of patient activities and observations and submit records to the appropriate clinical staff regarding patient's condition.
- Assist with scheduling patients and authorizations to support administrative staff as needed.
- Administers medications, injections, insulin, immunizations, perform finger sticks and functions in scope of license.
- Identifies patient goals, communicates, and collaborates with clinical team to support patient care plan.
- Performs clinical workflows.
- Monitor and record patient's vital signs, patient history and reason for visit.
- Explain treatments and procedures to patients and families.
- Assist with scheduling, referral and follow up appointments.
- Cross cover multiple practices.
- Assist with authorizations and administrative needs as needed.
Requirements
- High School Required
- AS Preferred
- 1-5 Years (Tier dependent) Required
- Annual Assessment - OHS Required
- Licensed Practical Nurse New York - New York State Board of Nursing Required
- National Council Licensure Examination for Practical Nurses (NCLEX-PN) or another license examination acceptable to NYSED Required
