About the Role

Join our dedicated team as a Case Manager I in the Treatment Apartment Program (TAP), an OMH-licensed scattered site housing initiative. This role is essential in providing person-centered case management and support services to individuals with Serious and Persistent Mental Illness (SPMI), including those with co-occurring disorders and housing instability. You’ll help participants build life skills, access housing and behavioral health services, and foster community and family connections—all while promoting wellness and stability.

Responsibilities

  • Implement individualized treatment and rehabilitation plans.
  • Provide consultation and foster peer support among participants.
  • Ensure safety and upkeep of residences, equipment, and supplies.
  • Assist individuals in securing and maintaining stable housing.
  • Conduct assessments and develop service plans.
  • Coordinate tenancy support, including conflict resolution and benefit navigation.
  • Maintain accurate documentation and comply with agency and regulatory standards.
  • Provide crisis support in various community settings.
  • Work closely with treatment teams, families, and external partners.
  • Build and maintain relationships with landlords and community providers.
  • Support internal and external reporting requirements.

Requirements

  • Bachelor’s degree in a human-services field, OR Associate’s degree + 3 years of experience, OR High school diploma + 5 years of experience in human services.
  • Minimum 2 years in case management or housing support for individuals with mental health/substance use needs preferred.
  • Prior experience in OMH Housing Programs strongly preferred.
  • Valid NYS Driver’s License and reliable transportation.

Work Type

  • Monday–Friday, 8:00 AM to 4:30 PM schedule, with flexibility as needed to meet program and participant needs.
  • Certain deadlines and unanticipated developments may require work during evenings, weekends.

Experience Level

  • Minimum 2 years in case management or housing support for individuals with mental health/substance use needs preferred.
  • Prior experience in OMH Housing Programs strongly preferred.

Education Level

  • Bachelor’s degree in a human-services field
  • Associate’s degree
  • High school diploma

Salary/Compensations

  • $21.00 - $25.00 an hour
