About the Role
Provide care coordination and health promotion services to individuals with severe mental illness and/or chronic health conditions to prevent costly hospitalizations and emergency room visits. Assist individuals in obtaining benefits and coordinate access to behavioral, medical, and social services.
Responsibilities
- Provide care coordination and health promotion services to individuals throughout Long Island.
- Assist individuals in obtaining Food Stamp and Social Security benefits.
- Oversee and coordinate access to all services (behavioral, medical health and social service supports).
- Complete all required documentation according to standards and no later than 24 hours of service delivery.
Requirements
- Minimum of a Bachelor’s degree with at least 2 years relevant experience serving children.
- Registered Nurse with 2 years relevant experience serving children.
- Master’s Degree with 1-year relevant experience serving children.
- Clean, valid NYS Driver’s License.
- Fingerprinting, criminal record check, and all necessary clearances from the Justice Center.
