About the Role

The Community Health Worker provides culturally appropriate health education and information to help community members access necessary care.

Responsibilities

  • Collaborate with the care team to provide patient-centered care.
  • Provide cultural mediation between community members and CommUnityCare.
  • Empower patients and families to actively participate in their care management.
  • Assist patients and families in setting personal goals and attending appointments.
  • Coach patients in effective health condition management and self-care.
  • Identify patient needs for internal and external referrals to social services and other resources.
  • Serve as a liaison between patients and healthcare systems.
  • Interface with providers and the clinic team to ensure patient adherence to treatment plans.
  • Act as a patient advocate at medical or social service appointments to bridge system gaps.
  • Follow up with patients via phone calls, home visits, and other settings as per program guidelines.
  • Maintain ongoing communication with community partners and promote safety net programs at community events.
  • Advocate for individual and community health needs and perform outreach activities.
  • Maintain privacy and confidentiality of patient medical records and visits.
  • Ensure all patient care and administrative tasks comply with regulatory and accreditation standards.
  • Develop and maintain positive internal relationships with colleagues, managers, and staff.
  • Interact respectfully and collaboratively with patients and their families.
  • Perform other duties as assigned.

Requirements

  • Graduated from a high school or equivalent.
  • One (1) year of experience working within a community health setting in a healthcare role or a community-based human service agency as a Community Health Worker (CHW), health educator, community outreach worker, or community organizer.
  • Certified Community Health Worker (CHW) or completion of an approved 160-hour competency-based Community Health Worker training program certified by DSHS within the first six (6) months of hire.
  • Valid Texas Driver’s License with no outstanding driving violations.
  • Access to personal vehicle.
  • Proof of $30,000/$60,000/$25,000 automobile liability insurance coverage.
