About the Role

Eddy Village Green has a rewarding opportunity for a full time Case Worker! The Case Worker will provide services to residents and families. The Case Worker will be involved with interdisciplinary and interdepartmental programs as they relate to the residents' quality of life.

Responsibilities

  • Assists families in supporting the resident and functions as an advocate for the resident to assure resident rights.
  • Assists residents in meeting and solving their social as well as economic needs and problems.
  • Provides personal counseling.
  • Develops psychosocial and behavioral care plans.
  • Responsible for referrals to appropriate agencies or professionals.
  • Acts as liaison between resident and community service providers.
  • Will facilitate discharge planning.

Requirements

  • Minimum of 1 year experience in skilled nursing facility/nursing home or senior/geriatric experience required.

  • Day Shift

Education Level

  • Bachelor's degree in Social Work is required.

Salary/Compensations

  • $23.40 - $33.69 per hour
