About the Role

Premier Health Partners, P.C., a partner of Erie County Medical Center (ECMC), seeks a dedicated and compassionate Licensed Practical Nurse (LPN) to provide quality patient care and assist with medical procedures. This role is vital in advancing wellness through trusted relationships, innovation, and patient-centered care.

Responsibilities

• Prepare patients for examination by performing preliminary physical tests, including manual blood pressure, height, weight, BMI, and pulse.
• Assist with patient assessments and document medical histories.
• Maintain patient care equipment.
• Observe and monitor patient conditions, documenting findings according to nursing standards.
• Maintain a safe, secure, and healthy work environment by adhering to internal and legal regulations.
• Assist with examinations and in-office treatments by providing necessary instruments and materials to providers.
• Administer immunizations and injections; may perform EKG testing, pulse oximetry, and phlebotomy.
• Transport patients in various healthcare settings.
• Educate patients and families on health conditions, treatment plans, and preventive care.
• Ensure accurate documentation of patient care in Electronic Medical Record (EMR) systems.
• Deliver exemplary customer service to ensure a positive organizational experience.
• Perform other duties as assigned by management.

Requirements

• Possession of a license and current registration to practice as a Licensed Practical Nurse by the New York State Education Department.
• 1-2 years of nursing experience in an ambulatory care setting is preferred.

Skills

• Strong clinical skills, including vital signs, medication administration, patient chart updates, and assisting with minor procedures.
• Proficiency in Electronic Medical Records (EMR) and other healthcare technology.
• Ability to perform patient assessments, develop care plans, and manage follow-up care.
