About the Role

The Public Health Specialist links and retains individuals living with HIV or at high risk of infection in health care, supporting medication adherence. This role involves confidential testing, STI screenings, and service coordination across a 15-county region, prioritizing candidates skilled in working with stigmatized communities and knowledgeable about local resources.

Responsibilities

  • Uphold best practices regarding confidentiality in all duties and communications.
  • Engage HIV+ and high-risk individuals who are out of care or inconsistently engaged, using evidence-based techniques to support improved health outcomes.
  • Identify and reduce barriers to care by facilitating access to needed services, including home visits or community-based meetings.
  • Provide confidential HIV/syphilis/HCV testing and STI screenings in targeted community venues.
  • Collaborate with Public Health Support Specialists to link clients to medical care, treatment, and supportive services.
  • Distribute syringes and connect clients with harm reduction services.
  • Integrate the Transtheoretical Model of Behavior Change into program delivery.
  • Lead multidisciplinary case conferences, coordinating with providers to promote engagement and retention in care.
  • Advocate on behalf of clients to secure services that support independence and self-management.
  • Maintain complete and accurate documentation in line with NYS regulations and agency protocols.
  • Track and report data as required.
  • Perform additional duties as assigned.

Requirements

  • Valid driver’s license and reliable transportation required.
  • Ability to work flexible hours, including evenings and weekends.

  • 15-county region

Work Type

  • Full-time
  • 37.5 hours per week

Experience Level

  • Relevant human services experience preferred
  • Familiarity with target populations and service systems strongly preferred

Education Level

  • Associate’s degree in Human Services, Public Health, or related field
  • Bachelor’s preferred
