About the Role
Oversee and coordinate patient recovery plans in a home care setting, providing advanced clinical services, patient assessment, and medication management while ensuring high-quality care and positive clinical outcomes.
Responsibilities
- Create and update comprehensive, interdisciplinary patient-centered home health plans of care in collaboration with physicians.
- Case manage assigned patients to achieve positive clinical outcomes and minimize rehospitalization risks.
- Support agency financial goals.
- Demonstrate expert knowledge in community health nursing.
- Coordinate care activities and share findings with families and professional caregivers.
- Review patient insurance information.
Requirements
- Current NYS RN license and registration.
- Minimum of one year of acute medical-surgical nursing experience.
- Valid driver license and access to a car.
- Bachelor’s degree in nursing preferred.
- CHHA experience preferred.
- Bilingual English/Spanish skills preferred.

- USD $112,000.00 - $134,100.00 per year
