About the Role
Assist Unit Leadership in assessing patient’s functioning, developing and implementing treatment plans and services as patient’s needs warrant.
Responsibilities
- Provides direct care services to patients individually and in groups, including activities of daily living, social, and psychiatric rehabilitation per treatment plan.
- Participates in the development of a comprehensive treatment plan by sharing patient needs and wants.
- Documents and communicates patient response to treatment effectively, accurately, and in a timely manner.
- Provides comprehensive care to patients professionally and compassionately, taking appropriate actions and communicating findings to the multidisciplinary team.
- Intercedes on behalf of customers to ensure they have access to required services.
- Participates in the peer review process and utilizes quality improvement techniques.
- Provides feedback, maintains professional relationships, and offers program and patient-specific feedback and recommendations as a member of the multidisciplinary team.
- Enhances professional growth and development through educational programs, current literature, in-service meetings, and workshops.
- Understands and practices established RGHS policies, procedures, goals, standards of care, quality improvement activities, safety, and environmental and infection control.
Requirements
- Minimum of 2 years of experience in a Human Services related field, medical related field, customer service, or client/patient interfacing experience required.
- Associate’s degree required.
- Bachelor’s degree preferred.
