About the Role

As a Field Care Supervisor at Sagecare, you will support the delivery of the highest quality care support services by assessing and reviewing individual services, and practically supervising, instructing, and supporting care workers. This role offers a meaningful and rewarding career where every day presents new challenges and opportunities.

Responsibilities

• Support the delivery of high-quality care support services.
• Assess and review individual services.
• Provide practical supervision, instruction, and support to care workers.
• Work with Care Coordinators to plan effective care worker rotas.
• Conduct regular reviews of individual services, including needs and risk assessments.
• Participate in multi-disciplinary reviews of service users’ needs.
• Shadow new care workers in the field to ensure competence.
• Perform on-site observation and evaluation of care worker practice.
• Spend the majority of working time in the community, with some office-based administrative work.

Requirements

• Experience in carrying out initial risk and needs assessments for individual service users.
• Ability to work in partnership with service users, families, carers, advocates, and healthcare professionals.
• A driving licence is essential.
• Own transport is essential.
