About the Role

FOX Rehabilitation is seeking passionate, licensed Speech Language Pathologists to deliver one-on-one care to older adults in their homes or senior living communities. This role focuses on improving patient mobility, independence, and quality of life through a Geriatric House Calls model, offering autonomy and flexibility in scheduling.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in private residences and senior living communities
• Perform evaluations and develop individualized plans of care
• Deliver evidence-based treatment to support patient wellness and functional independence
• Document patient progress effectively within the EMR system
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth and virtual care services when appropriate

Requirements

• Valid Speech Language Pathology license in the state of practice or eligibility to obtain
• Degree from an accredited Speech Language Pathology program
• Basic computer and EMR proficiency
• Current CPR certification
• Current Certificate of Clinical Competence (ASHA CCC)
• Ability to exert medium physical effort including lifting 25-50 lbs
• Ability to perform CPR and assist with patient transfers and bed mobility
• Ability to stand, walk for extended periods, climb stairs, and perform therapeutic activities
• Valid driver's license and ability to travel to patient homes and communities
• Ability to wear PPE when required
