About the Role
Powerback is a leader in physical, occupational, speech, and respiratory therapies, helping older adults stay active and thrive. This role provides outpatient rehabilitation services in an in-home setting for the older adult population, offering a flexible schedule and work environment.
Responsibilities
- Provide outpatient rehabilitation services in an in-home setting for the older adult population.
- Evaluate and treat patients in their home environment.
- Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation.
- Identify the need for and make referrals to other disciplines.
- Implement individualized therapy programs to restore, reinforce, and enhance performance.
- Instruct and educate patients and caregivers in skills and functions essential for promoting independence.
- Direct activities to promote and maintain health.
- Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as needed.
- Adhere to department and patient treatment schedules, documentation requirements, and work-related meetings.
- Attend and contribute to patient care, staffing conferences, and other related meetings.
- Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen.
- Adapt program and/or plan of care according to individual patient needs.
- Promote a safe environment and enforce the elimination of fire and safety hazards.
- Order supplies and equipment as necessary.
- Clean, maintain, identify, and report the need for repair of equipment.
- Prioritize patient service, ensuring high-quality care in a compassionate atmosphere.
- Screen, examine, and evaluate patients, including history, systems review, and application of tests and measures.
- Synthesize information and interpret examination findings to establish a diagnosis and plan of care.
- Develop appropriate treatment goals and methods in collaboration with patients and caregivers.
- Implement the physical therapy treatment plan and complete all related documentation.
- Provide therapeutic exercise, functional training, manual therapy, airway clearance techniques, integumentary repair and protection, and use of modalities.
- Provide health and wellness education, including chronic disease management and falls assessment.
- Perform case management in the home and community.
- Design, fabricate, fit, and/or apply adaptive devices and train patients in their use.
- Select appropriate activities suited to the individual's capacity, intelligence level, and interest.
- Instruct patients in skills and techniques for mobility activities.
- Evaluate patient progress, attitudes, and behavior related to rehabilitative potential.
- Perform routine reexamination as needed to modify/progress the plan of treatment.
- Incorporate health and wellness programming into treatment plans.
- Perform home and community mobility assessments and provide recommendations.
- Perform other related duties as required.
Requirements
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence.
