About the Role

The FTC Facilitator serves as an integral member of the Prevention team, ensuring family voice and choice, and integrating core principles and practices of Mobility Mentoring into work and planning with families. The role supports family stability and growth by connecting them with resources and services to achieve their goals.

Responsibilities

• Facilitate FTCs for families and staff to review services, assess progress, and develop action plans.
• Utilize the six-stage model for FTCs and support the use of Mobility Mentoring.
• Maintain an active and flexible FTC schedule, responding to crisis intervention and changing family needs.
• Serve as an active member of the interdisciplinary team, attending case conferences and reviewing case records.
• Conduct home visits to hold conferences when needed.
• Submit accurate and timely documentation summarizing each FTC in PROMIS and Connections databases.
• Conduct follow-up reviews to ensure tasks are occurring as planned.
• Facilitate ongoing and/or short-term participant groups or workshops.
• Attend and participate in all required meetings, case consultations, supervision, and training.
• Perform other duties as assigned by the supervisor.

Requirements

• Masters degree in Social Work or two (2) years casework experience and one (1) year group work experience.
• Minimum 1 year Supervisory experience is a plus.
• At least two years of work experience in a child welfare-related setting facilitating groups and/or team meetings.
• Excellent group facilitation and communication skills.
• Ability to work well on a team.
• Organized and able to manage multiple responsibilities concurrently, including during a crisis.
• Proficiency in Microsoft Office (Outlook, Excel, and Word).
• Familiarity with Connections and PROMIS databases is a plus.
• Commitment to working from a strength-based, youth and family development, and trauma-informed perspective.
• Dedicated to being a team player and active contributor to achieve the vision and mission of the program and agency.
• Bilingual in English/Spanish is a plus.
