About the Role

Responsibilities

Provide home and community-based services to a reduced caseload of clients with varying needs, with a minimum of two visits per month and additional contact as clinically appropriate.
Collaborate with service providers, maintain complete care management health records, and offer crisis intervention, prevention, and resolution.
Participate in on-call rotation duties and provide coverage as needed.
Conduct ongoing outreach, including program presentations.
Develop and nurture new and existing program partnerships with key stakeholders, community organizations, and hospitals to build a robust referral network.
Receive ongoing training and professional supervision.
Perform other duties as assigned by the Program Director.

Requirements

BA in a related field.
At least two years of experience working with children, preferably in a mental health setting.
Demonstrated ability to engage hard-to-reach populations.
Strong interpersonal skills, including tact, diplomacy, patience, professionalism, and social perceptiveness.
Excellent verbal and written communication skills.
Bilingual in Chinese or Spanish is a plus.

The target population is: children/youth ages 0 - 21 who have: two or more chronic conditions or one single qualifying condition of Serious Emotional Disturbance (SED) or HIV/ AIDS or Trauma/at risk for another condition.
