About the Role
Empower people to live their best lives by providing direct care services, accompanying individuals on medical appointments, and advocating for their healthcare needs within a large nonprofit health and human services organization.
Responsibilities
- Accompany persons served on medical appointments and advocate for appropriate medical services.
- Provide direct care services to persons served.
- Act as a liaison between the residence and medical facilities, hospitals, laboratories, and pharmacies.
- Ensure medical, dental, or other therapeutic appointments are carried out as required.
- Collaborate with the Residence Nurse and Residence Manager to ensure proper medical follow-up.
- Advocate for persons served needs with physicians, lab technicians, psychiatrists, dentists, and other medical providers.
- Ensure clinical consults are completed appropriately before and after medical appointments.
- Safely operate agency vans and/or vehicles.
- Communicate immediate medical concerns to the Residence Manager and/or Residence Nurse.
- Participate in weekly consult review meetings.
- Obtain and place medical documentation (e.g., Blood work, EKG, X-ray reports) in binders for consultations, hospitalizations, or placements.
- Understand, articulate, and document specific medical information.
- Assist in training persons served in activities of daily living (ADL) and development of self-help and social skills.
- Perform ADL functions for individuals requiring total support (e.g., diaper changing, ambulation, G-Tube feeding, transferring with Hoyer lift).
- Implement goals to maximize each person’s independence, including budgeting, grooming, nutritional planning, public transportation use, community resource utilization, shopping, and housekeeping.
- Enter person served data documentation in the EHR system.
- Oversee all medication as per WellLife Network’s Medication Administration Policy, including the computerized EMAR system.
- Communicate relevant information to other staff or supervisors verbally and in writing.
- Complete appropriate logs and incident reports.
- Physically intervene with persons served when they may cause physical injury to self or others.
- Assist individuals in responding to potentially dangerous situations.
- Administer emergency first aid.
- Maintain confidentiality at all times.
- Perform other duties as requested.
Requirements
- Computer use/ability is required.
- Must be able to safely operate agency vans and/or vehicles as per the agency driving policy.
- Must be able to understand, articulate, and document specific medical information.
- Required to display proficiency while entering person served data documentation in the EHR system (Electronic Health Record).
- Required to display proficiency while entering person served data documentation in the EHR system (Electronic Health Record).
- Must be able to physically intervene with the person served when the person may cause physical injury to self or others.
- Must successfully complete and perform all techniques required in AMAP, SCIP, CPR and First Aid.
- Experience working with the developmentally disabled and/or experience in the medical field.
- A valid NYS Drivers license is required.
